Judy, a SIHO member, was hospitalized with severe pneumonia. In her absence, her husband, John, juggled caring for the kids, taking care of household chores and his full-time job. Adding to that stress was concern for his wife and the care she needed when released from the hospital. Upon discharge, Judy needed a hospital bed and oxygen.
The Solution – Proactive Approach
At SIHO, we offer on-site review of all inpatient stays at Schneck Medical Center in Seymour, Columbus Regional Hospital and Bloomington Hospital. We also do phone reviews for any patients in hospitals throughout southern Indiana. This affords our Nurse Case Managers the opportunity to be available to our members for discharge planning needs. We work closely with our members, physicians and nurses at each of the hospitals to make sure our members’ needs are met.
In Judy’s case, we worked with her physician, her nurse at the hospital and the local equipment company to make all the arrangements. We verified her health benefits and made sure the right vendor was used to get the most reasonable price and best use for the family. Judy returned home with a smooth transition. Her medical needs were prepared in advance for her arrival home and John could continue working and help Judy focus on their family.
