A SIHO Case Manager had a patient with high blood pressure. Making her routine phone calls to her patients she asked Mike, a SIHO member, how he was doing. He said great! Mike reported he was doing exactly as his doctor instructed, to take his blood pressure everyday and keep a log, documenting it, to bring with him the next time he had an appointment. The Nurse Case Manager asked him what his blood pressure was for the day and he told her 220/110.
The Solution – Regular Contact
SIHO’s Case Managers proactively contact members to ensure proper routine care is selected. In this case, Mike didn’t completely understand the seriousness of his case and the critical need to contact his doctor. The case manager instructed him to call his doctor right away. As a result, his physician made some medication changes which brought his blood pressure down to where it needed to be. This avoidance of a potential stroke, or life threatening emergency was simply due to a routine phone call that our Nurse Case Manager made.
